Fasting sugar and HbA1c are not the same test

Families wave one number and call it ‘the sugar’. There are several different sugar tests, and they answer different questions.

In plain language

A fasting sugar is a snapshot after about eight hours without calories. A post-meal sugar shows how you handled that plate. HbA1c estimates the average over about three months, because sugar sticks to red blood cells for their lifespan. Anaemia, recent bleeding, and some haemoglobin conditions can make HbA1c lie. That is why a clinician reads the tests together.

Signs worth noticing

Do not compare today’s fasting number with last year’s HbA1c and declare yourself cured or doomed.

What usually sits underneath

Meal timing, illness, steroids, poor sleep, and missed tablets move the snapshot. The three-month average moves more slowly, which is why it is used to judge a plan.

Care that is reasonable at home

  1. Ask which test was ordered before you fast all morning for the wrong one.
  2. For fasting tests, water is allowed. Tea is not.
  3. Bring the old sheet. A trend of 6, 7, then 8 means more than a single 8.
  4. If the two tests disagree, let the clinician explain. Do not average them yourself.

Home glucometers need control solution or a comparison with the lab now and then. A cheap strip can be consistently wrong.

Which test is best?

They answer different questions. A new diagnosis often uses more than one. Follow-up often uses HbA1c plus home checks if you are on treatment.

Can I eat before HbA1c?

Usually yes. Confirm when you book the sample, especially if other tests are on the same form.

How often?

Often every three months when things are changing, and less often when they are stable. Your own plan wins.
