A red eye: allergy, infection, or a warning

A itchy red eye with a watery discharge in both eyes is often allergy or a viral conjunctivitis. A painful red eye with blurred vision is a different illness and should be seen promptly.

In plain language

Viral and allergic conjunctivitis are common, uncomfortable, and usually not dangerous. Bacterial conjunctivitis produces more pus. A red eye with pain, light sensitivity, reduced vision, or a halo around lights can be ulcer, uveitis, or acute glaucoma. Those are urgent. Steroid drops can make a viral ulcer much worse. Do not use a leftover steroid because it helped a relative’s itch.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

Both eyes itch and water, and vision is normal.
A sticky eye in the morning that wipes clear and sees well.
Pain, light hurting the eye, or vision down.
A injury, a chemical splash, or a contact lens wearer with a red painful eye.

What usually sits underneath

Viruses and allergens inflame the surface. Contacts, when worn too long or in a pool, invite ulcers. Hands spread viral conjunctivitis through a household quickly.

Care that is reasonable at home

Wash hands. Do not share towels.
Use lubricating drops if vision is normal and pain is absent.
Stop contact lenses until a clinician has said the eye is well.
Do not patch a red eye at home.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

When waiting is a bad idea

Any drop in vision, significant pain, or a pupil that looks different.
A chemical in the eye. Rinse with plenty of clean water and come urgently.
A newborn with a red or discharging eye.

Questions people ask

Is it contagious?

Viral and bacterial conjunctivitis often are. Allergy is not. Handwashing is the practical response.

Will breast milk or rose water help?

Do not put them in the eye. They are not treatments and they can infect it.

How long should a simple red eye last?

A few days. If it is worsening or vision changes, the ‘simple’ label was wrong.
