Sore throats and a voice that has gone
Most sore throats are viral, hurt for a few days, and do not need an antibiotic. A throat that is extremely painful, with drooling or difficulty breathing, is not that illness.

In plain language
Viral throats come with a cold: runny nose, cough, and a scratch. Streptococcal throats are more likely if there is fever, tender neck glands, and no cough, especially in a child. A clinician can decide whether a test or an antibiotic is useful. Whispering strains the voice more than quiet speech. Shouting over a wedding sound system is how teachers and singers lose a week.
Signs worth noticing
These are the clues families usually notice first. One of them, on its own, is not a diagnosis.
- A scratch with a cold, swallowing still easy.
- Fever and a very red throat without a cough.
- A voice that is hoarse after shouting.
- Drooling, a muffled voice, difficulty breathing, or a throat on one side that is exquisitely painful.
What usually sits underneath
Viruses inflame the lining. The voice box swells when it is abused or infected. Acid reflux reaches the throat at night and causes a morning soreness that people mistake for a infection. Tobacco keeps the lining in a state that should not be ignored if hoarseness lasts.
Care that is reasonable at home
- Fluids, rest, and a pain approach that is safe for you.
- Speak quietly or not at all for a day if the voice is the problem. Do not whisper.
- Treat reflux and tobacco if the soreness is every morning.
- Do not take an antibiotic ‘to be safe’ for a obvious cold.
These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.
When waiting is a bad idea
- Breathing difficulty, drooling, or a one-sided throat with fever.
- Hoarseness lasting more than three weeks, especially in a smoker.
- A child with a sore throat who is not drinking.

Questions people ask
Are lozenges useful?
They soothe. They do not treat a bacterial throat. Sugar-free ones are kinder to teeth.
Can I gargle?
Warm salt water is reasonable. Do not gargle a strong antiseptic that burns.
Is ice cream harmful?
Cold can soothe. It is not a cure and not a crime.
