Sprain or fracture after a twist
Swelling does not tell you which is which. Inability to take four steps, or a bone that is tender in a precise spot, should be seen.

In plain language
A sprain is a torn ligament. A fracture is a broken bone. Both swell. In the first hours, rest the part, cool it with a cloth-wrapped ice pack for short spells, compress lightly, and raise it. Do not massage a fresh injury. Do not walk on an ankle that will not take weight just to prove a point.
Signs worth noticing
These are the clues families usually notice first. One of them, on its own, is not a diagnosis.
- You can walk, and the pain is over the soft side of the joint.
- You cannot take four steps.
- The bone itself is sharply tender, or the limb looks bent.
- The skin is broken, or the toes or fingers beyond the injury are pale, numb, or cold.
What usually sits underneath
Ligaments tear when a joint is forced past its range. Bones break at predictable points in falls on the outstretched hand or twisted ankle. Some fractures are small and only show on a film.
Care that is reasonable at home
- Stop using the limb if weight hurts sharply.
- Ice through cloth, twenty minutes at a time, not directly on skin.
- Raise the part. A sling or a pillow works.
- Come for examination the same day if you cannot use it, if it looks deformed, or if you are unsure.
These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.
When waiting is a bad idea
- A limb that looks crooked, or bone visible through skin.
- Numbness, blueness, or a pulse you cannot feel beyond the injury.
- Pain out of proportion, especially in a child.

Questions people ask
Can I strap it at home?
A simple support is fine for a mild sprain you can walk on. A fracture needs the right immobilisation, not a tight cloth that hides swelling.
How long does a sprain hurt?
Often a few weeks. Pain that is not improving, or a joint that keeps giving way, should be reviewed.
Is heat useful on day one?
No. Heat and massage in the first day increase swelling. Later, once a clinician has said it is a sprain, gentle movement matters.
