Abdominal pain that should not be watched overnight

Gas pain moves, rumbles, and eases when you pass wind. Pain that is steady, severe, and accompanied by fever or vomiting does not get a home trial.

In plain language

The abdomen holds gut, gallbladder, appendix, urinary tract, and, in women, the ovaries and pregnancy. Location and company matter. Central pain that moves to the right lower side, with fever and loss of appetite, is a classic appendicitis story. One-sided lower pain in a woman who could be pregnant is ectopic until proved otherwise. Pain that is out of proportion to the examination is sometimes a surgical emergency.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

Crampy pain with diarrhoea after a meal, and a soft belly.
Pain that settles after passing stool or wind.
Steady pain that is getting worse over hours.
A rigid belly, fainting, or vomiting green fluid.

What usually sits underneath

Indigestion and muscle wall pain are common. Stones, appendicitis, ulcers that have perforated, kidney stones, and pregnancy complications are the ones that harm people who were told to wait until morning.

Care that is reasonable at home

For mild crampy pain with a soft belly and no fever, sips of fluid and a pause are reasonable.
Do not take a painkiller and a sleeping tablet and hope. Painkillers can hide a surgical abdomen.
Do a pregnancy test if pregnancy is possible, before any other theory.
Come in if you are unsure. Leaving a belly unseen is worse than a wasted visit.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

When waiting is a bad idea

Severe or worsening pain, especially with fever, vomiting, or a swollen belly.
Possible pregnancy with pain or bleeding.
Chest pressure that you are calling gas.

Questions people ask

Can I take an injection for pain at a chemist?

No. An injection without an examination can mask a perforation or an ectopic pregnancy.

Is the appendix always on the right?

The pain often starts in the centre and then moves to the right lower side. Unusual positions exist. The trend of the pain matters.

What if the ultrasound is booked for tomorrow?

If the pain is severe now, you need an examination now. The scan is not the first step.
