Diabetes and the eyes: why ‘I can see fine’ is not a test

The retina can be badly damaged while you still read the newspaper. Waiting for symptoms is how people lose the sight that screening would have saved.

In plain language

Sugar weakens the small vessels at the back of the eye. They leak, they close, and sometimes fragile new vessels bleed. A dilated eye check looks at that, which a spectacle shop does not. Early changes are treated more easily than a bleed.

Signs worth noticing

Blurred vision that fluctuates with your sugars is often the lens swelling, and it may settle once sugars are steadier. Do not buy expensive glasses in the middle of a sugar crisis.

Floaters, a curtain, or a sudden shower of spots. That can be a bleed.
Straight lines that look bent.
Blur that is different between the two eyes.
No symptom at all, which is still a reason for a scheduled check.

What usually sits underneath

Years of high sugar, high pressure, kidney disease, and pregnancy speed the damage. The longer the diabetes, the more the eyes need a look, even when control is now good.

Care that is reasonable at home

Ask for a retina check at diagnosis of type 2, then on the schedule you are given, often yearly.
Keep pressure and sugar in the range your clinician set. Eyes follow those numbers.
Do not use over-the-counter steroid drops because a relative had itching.
If vision drops suddenly, treat it as urgent. Do not sleep on it.

Drops that dilate the pupil will blur you for a few hours. Do not plan to ride a two-wheeler home alone after that test.

When waiting is a bad idea

Sudden loss of vision, a curtain, or many new floaters.
Eye pain with redness and halos, which may not be diabetes but is still urgent.
Pregnancy with diabetes. The eye checks are more frequent.

Questions people ask

Does good sugar repair the retina?

It slows further damage. It does not always undo scars. That is why the check happens before symptoms.

Can I skip it if my HbA1c is fine?

No. Duration still matters, and pressure matters too.

Will the check hurt?

Dilating drops sting briefly. The examination should not be painful.
