A smaller appetite can be normal. Losing several kilos without trying is not a virtue and not a normal age.

Muscle is lost with age, faster if protein and activity disappear. The person looks ‘fine’ and is actually weak. Teeth, depression, loneliness, medicines, and illness all reduce intake. A plate with dal or curd, some vegetable, and a starch, twice a day, beats a single cup of tea and a biscuit. Unintended weight loss needs a clinician to look for thyroid disease, diabetes, depression, cancer, and a mouth that hurts.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

What usually sits underneath

Taste dulls. The effort of cooking for one feels pointless. Acid tablets and a dry mouth change eating. None of this is fixed by a protein powder if the person is depressed or cannot chew.

Care that is reasonable at home

  1. Check the teeth and the denture.
  2. Serve a protein food at each meal: dal, curd, egg, paneer, or milk.
  3. Eat with someone when you can. Appetite is social.
  4. Weigh once a month. Write it down.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

Are supplements required?

Sometimes vitamin D or B12, if a test shows a need. A expensive powder is not the first step.

Look at mood, mouth pain, and constipation. Forcing a large plate fails. A small plate they accept is better, and a clinician should know.
