Piles: itch, bleed, and the habit underneath

Piles are swollen cushions in the anal canal. They are common. Fresh blood on the paper still deserves a clinician at least once, because piles are not the only cause of bleeding.

In plain language

Straining, constipation, pregnancy, and long sitting raise pressure in the cushions. They then bleed bright red, itch, or prolapse. Pain that is sharp and tearing is often a fissure, a split in the skin. A lump that is suddenly purple and agonising can be a clotted external pile. Cancer and colitis also bleed, which is why a new pattern is examined.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

Bright red blood on the stool or the paper.
Itch and a feeling of moisture.
A lump that comes down and goes back.
Dark blood, weight loss, or a change in the stool shape.

What usually sits underneath

Hard stool and straining are the engine. Treating the pile and ignoring the constipation brings it back. Pregnancy piles often improve after delivery if the bowel habit is gentle.

Care that is reasonable at home

Soften the stool with water, fibre, and a footstool in the toilet.
Do not sit and strain. If nothing happens, get up and walk.
Sit in warm water for comfort during a flare. Pat dry.
Use the cream you were prescribed for a short flare, not for the rest of your life.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

When waiting is a bad idea

Heavy bleeding with dizziness.
A painful black lump at the anus, or fever around a swelling.
Bleeding that is new after fifty, or bleeding plus weight loss.

Questions people ask

Is surgery always required?

No. Most piles settle with stool softening and time. Surgery is for prolapse or bleeding that does not settle.

Can I use a relative’s steroid cream?

Steroid creams thin the skin if used for weeks. Ask before you continue one.

Does spicy food cause piles?

Spice irritates some people during a flare. Constipation causes the condition.
