Feet that swell by evening

Tight shoes at 8 pm are common in people who stand all day. Swelling that climbs, comes with breathlessness, or is there from morning is not the same thing.

In plain language

Fluid leaves tiny vessels and sits in the soft tissue of the ankles when you are upright. Varicose veins, heat, salt, pregnancy, and some blood-pressure tablets do this. Heart, kidney, and liver disease do it too, usually with other clues.

Signs worth noticing

Press a finger over the shin for a few seconds. A dent that stays is worth mentioning. So is swelling of only one leg.

Both ankles, worse at night, better by morning, in a person who stands at a shop counter.
One leg, suddenly swollen, painful, and warm. That can be a clot and should not be massaged.
Swelling plus breathlessness when lying flat, or waking up gasping.
Puffiness of the face in the morning, with frothy urine. Think kidneys, not just tired feet.

What usually sits underneath

Gravity, leaky leg veins, salt, pregnancy, and medicines such as some calcium-channel blockers are frequent. Heart failure, low protein, kidney disease, and clots are the reasons not to shrug at a new pattern.

Care that is reasonable at home

Put the legs up when you sit in the evening. A stool is enough.
Walk during the day so the calf muscle pumps fluid back up.
Do not stop a pressure tablet because of mild ankle swelling. Ask. There is often an alternative.
Weigh yourself a few mornings if swelling is recurring. A jump of a couple of kilograms in days is a fact, not a feeling.

Compression stockings help some vein problems and harm some artery problems. Do not buy a tight pair off a rack if you have calf pain on walking.

When waiting is a bad idea

One leg swollen and painful, especially after a long journey or surgery.
Breathlessness, chest pain, or swelling up to the thighs in a few days.
Swelling in pregnancy with headache or visual change.

Questions people ask

Is swelling always heart failure?

No. Standing, veins, and medicines are common. Breathlessness and sudden weight gain are the company that worries clinicians.

Can I take a water tablet from a relative?

No. Water tablets shift salts in the blood and can be dangerous without a reason and a blood test.

Does elevation work?

For simple dependent swelling, yes, in the evening. It will not fix a clot or heart failure.
