How to bring an older person to OPD

The attendant often speaks for ten minutes while the patient sits unheard. Decide in advance who will start, and let the patient speak if they can.

In plain language

Bring the medicine bag, the folder, a list of symptoms in order of worry, and a snack and water. Say the worst fact first: a fall, blood, breathlessness, confusion. Then the background. Ask for the prescription to be written in a hand you can read, with times of day. Before you leave, confirm what to stop, what to start, and what should bring you back. If the person is confused today and was not last week, say ‘this is new’.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

What usually sits underneath

Older patients under-report. Companions over-report. The clinician needs both, and needs them labelled. A new confusion is a medical symptom. A long grudge about food is not.

Care that is reasonable at home

  1. Write the two questions.
  2. Carry a phone list of allergies.
  3. Plan the toilet and the water. A long wait without either causes a different illness.
  4. On the way home, read the new list aloud so the household agrees.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

Questions people ask

Should the patient stay in the vehicle?

No, if they can come in. Examination needs the person. If they cannot walk, say so. Do not leave them in a closed car in summer.

What if they refuse the visit?

If they are safe and merely unwilling, try another day. If they are confused, bleeding, or breathless, the refusal is part of the illness. Seek help.

Can two relatives come in?

One companion who knows the facts is better than four who argue.
