Allergic reactions: rash, wheeze, and collapse
A rash with itching after a new medicine or a food is a reason to stop that exposure and ask for advice. A rash with wheeze, swelling of the tongue, or collapse is a emergency.
This page is general education for families in Gurugram. It is not a diagnosis, a medicine list, or a reason to skip a clinician. Doses are deliberately not given.
In plain language
Anaphylaxis is a severe allergic reaction. It can include hives, swelling of the lips or tongue, wheeze, vomiting, and a drop in blood pressure. Common triggers are medicines such as penicillin, insect stings, nuts, and some foods. If the person has a prescribed adrenaline auto-injector, use it as you were trained, into the outer thigh, and still go to hospital. Antihistamine tablets are not enough for anaphylaxis. Do not try to induce vomiting.
Signs worth noticing
These are the clues families usually notice first. One of them, on its own, is not a diagnosis.
- A localised itchy rash, the person otherwise well.
- Swelling of the lips with a normal voice and normal breathing. Still be seen.
- Wheeze, a hoarse voice, difficulty breathing, or fainting.
- A known severe allergy and a possible exposure, even before the symptoms peak.
What usually sits underneath
The immune system releases a flood of chemicals. Blood vessels leak and airways narrow. It can return in a second wave, which is why a period of observation is required even if the person improved.
Care that is reasonable at home
- Stop the suspected food or medicine.
- If they have an adrenaline pen and this is anaphylaxis, use it.
- Lay them down with the legs raised if they feel faint, unless they are vomiting or struggling to breathe. Then a sitting position may be easier.
- Call emergency help. Do not leave them.
These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.
When waiting is a bad idea
- Any breathing difficulty, collapse, or tongue swelling.
- A child with a known allergy and a possible exposure.
- A rash with blistering or with ulcers in the mouth after a medicine. That can be a severe drug reaction.

Questions people ask
Can I take a antihistamine and stay home?
For a mild itchy rash in a well person, a clinician may agree. If breathing or faintness is involved, no.
What if the pen is expired?
Use it if that is all you have in anaphylaxis, and still get help. Replace it this week.
Should the whole family avoid the food?
Only if they are allergic. Do label the patient’s food clearly so a well-meaning relative does not ‘just add a little’.
