Heavy bleeding: what to do before help arrives

Direct pressure stops most bleeding. A towel, a hand, and five uninterrupted minutes do more than ice water and panic. Peeking every thirty seconds restarts the clock.

In plain language

Press a clean cloth on the wound with the flat of your hand. If blood soaks through, add another cloth. Do not remove the first. Raise the part if you can do it without causing a other injury. A tourniquet is a last resort for a limb that is bleeding to death, and it causes harm if it is done badly. Nosebleeds are pinched at the soft part of the nose, leaning forward, for ten minutes. Leaning back swallows blood and hides how much there is.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

A cut that slows under steady pressure.
A nosebleed that stops with a proper pinch.
Blood that is spurting, or soaking a cloth in minutes.
A person who is pale, sweaty, and thirsty, or who has a head injury and is bleeding.

What usually sits underneath

Volume loss causes shock: fast pulse, cold skin, confusion. The person may still be talking. Internal bleeding — after a crash, into the thigh, or from an ectopic pregnancy — may show almost no blood outside and a person who is collapsing.

Care that is reasonable at home

Use pressure. Keep it on.
Call for help while someone else presses, if a second person exists.
Do not remove an object that is stuck in the wound. Press around it.
For a nosebleed, pinch and lean forward. Do not pack random cotton deep into the nose.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

When waiting is a bad idea

Bleeding that does not slow, a soaked cloth, or signs of shock.
Pregnancy with bleeding and pain or faintness.
A head injury, or bleeding on a blood-thinning medicine.

Questions people ask

Should I clean it first?

Not if the bleeding is heavy. Pressure first. Cleaning is for the bleeding that has already stopped.

Is ice better than pressure?

No. Ice is a extra. Pressure is the treatment.

What if I feel faint at the sight of blood?

Sit, and call someone else. A second fainted person does not help.
